The average BMI for men aged 60 to 69 is approximately 28.0, which sits in the overweight category. This reflects a broad population trend — most men in their 60s carry more weight than is considered optimal for long-term health.

💡 Average BMI tends to decrease slightly after age 70, partly because of natural muscle and weight loss with age (sarcopenia), not necessarily improved health. A lower BMI in older men is not always a positive sign if it reflects muscle loss.

Does Healthy BMI Change After 60?

The official WHO healthy BMI range of 18.5 to 24.9 does not change with age. However, research on older adults has found a more complex picture. Some studies suggest that men over 65 with a BMI of 25 to 27 may have slightly lower mortality risk than those at the very low end of normal weight. This is sometimes called the "obesity paradox."

The most important factor is body composition — specifically, maintaining muscle mass while keeping body fat, especially visceral fat, in check. Two men can have identical BMIs with very different health profiles depending on how much of their weight is muscle versus fat.

For men of Asian descent, standard BMI cutoffs underestimate health risk. Research consistently shows that Asian men develop cardiovascular and metabolic risk at lower BMI levels than Western populations. The recommended Asian-specific cutoffs are:

Key Health Risks for Men Over 60 at High BMI

Carrying excess weight after 60 increases risk of several serious conditions. The most significant for men in this age group are cardiovascular disease, type 2 diabetes, sleep apnoea, osteoarthritis in weight-bearing joints, and certain cancers including colorectal cancer.

Waist circumference is an important additional measure. For men, a waist above 94 cm (37 inches) indicates elevated risk, and above 102 cm (40 inches) indicates high risk — regardless of BMI.

Maintaining a Healthy Weight After 60

Weight management after 60 is harder for several reasons. Metabolism slows, muscle mass decreases (making it easier to gain fat at the same calorie intake), and hormonal changes affect fat distribution. The most effective strategies for men over 60 are:

One of the most important things to understand about BMI after 60 is that the same BMI can represent very different health states depending on body composition. Two men at BMI 26 — one with 20% body fat and strong muscle mass, one with 30% body fat and significant sarcopenia — have very different metabolic risk profiles.

After 60, men lose muscle mass at approximately 1–2% per year if not actively training. This means that even if the scale stays the same, the ratio of fat to muscle shifts unfavourably — a phenomenon sometimes called "sarcopenic obesity." The man at BMI 24 who hasn't exercised in 10 years may carry more visceral fat than his BMI suggests.

BMI 27 puts a man in the "overweight" category by standard WHO guidelines. However, some research suggests that for adults over 65, a BMI in the 25–27 range may be associated with better outcomes than the standard healthy range of 18.5–24.9. This is thought to be due to nutritional reserves during illness. That said, waist circumference and muscle mass are better indicators of health risk than BMI at this age. If waist circumference is under 94 cm and physical fitness is maintained, a BMI of 27 is unlikely to represent a significant health concern for most men over 60.
For most men over 60, preserving and building muscle mass (through resistance training and adequate protein) delivers more health benefit than weight loss alone. Muscle mass supports metabolic rate, bone density, functional independence, and cardiovascular health. A man who loses 5 kg of fat while maintaining muscle is in a far better health position than one who loses 5 kg of mixed fat and muscle. If weight loss is needed, prioritise preserving muscle by using a moderate deficit (300–500 kcal/day) with 1.6–2.0 g protein/kg/day and resistance training.
No — standard BMI categories (underweight, healthy, overweight, obese) are the same for all adults regardless of age. This is a recognised limitation: the same BMI in a 65-year-old represents a different body composition than in a 25-year-old due to age-related muscle loss and fat redistribution. Some researchers argue for age-adjusted BMI thresholds, but official guidelines (WHO, CDC, NHS) have not adopted this approach. In practice, clinicians often supplement BMI with waist circumference and other markers when assessing older adults.
